Physical Examination Form

This form should be completed by a physician and must be on file prior to the student athlete being eligible to participate in basketball or cheerleading activities, including practices. 

Eligibility and Authorization Statement

This form should be completed by the parent/guardian and signed by athlete and must be on file prior to the student athlete being eligible to participate in basketball or cheerleading activities, including practices.

Cheerleading Participation Agreement

These forms outline the rules for participating in athletics and must be signed by a parent and the student athlete prior to participating in basketball or cheerleading activities, including practices. 

MSHSAA Pre-Participation Documentation

This form is an annual requirement from MSHSAA.
